Open enrollment has come to a close. All previously enrolled employees who did not re-enroll during "open enrollment" will be deleted from the "Fund" effective October 30th, 2009.
Any employees who wish to enroll for the first time may do so by completing an enrollment application, attaching the required tax documents and any other documentation required per DCRF guidelines.
All employees enrolling must submit a copy of PAGE ONE of their 2008 IRS 1040 form and 2008 W-2 form. Married employees who file jointly must also submit their spouse's W-2. Employees who are married but filed separately must submit a copy of their spouse's W-2 and IRS 1040 form page one.

Employees and care providers must sign enrollment applications.
Employees who were formally enrolled but did not re-enroll during open enrollment September 09 must complete a new application and attach all required documentation. Former fund participants who did not re-enroll during open enrollment are not be eligible for retro active reimbursement for any months prior to receipt of the "new" enrollment application.
Effective November 1, 2009 a new enrollment application must be completed by all former and current employees who wish to be eligible for this contractual benefit.

Delays in submitting requested documents (2008 IRS 1040, 2008 W-2 or any other substantiating documentation will delay enrollment processing and ultimately effective date of enrollment.
